A 68-year-old man, a Canadian citizen with provincial health coverage, is admitted to a palliative care unit with metastatic pancreatic cancer. His estimated prognosis is weeks to a few months. Pain and cachexia cause constant suffering that he states is intolerable and cannot be relieved in a way he finds acceptable, despite optimized opioids, adjuvant analgesics, and nutritional support. Over the past week he has repeatedly and independently asked for medical assistance in dying (MAID), and he says no one is pressuring him. On examination he is alert, oriented, and cognitively intact, and he can explain his illness, the options, and the consequences of his decision. His medications include hydromorphone by continuous subcutaneous infusion, dexamethasone, and pancreatic enzyme replacement.
