A 71-year-old woman presents with 5 weeks of bilateral red, gritty eyes with a foreign-body sensation that is worse in the evening. Eight weeks ago she started immediate-release oxybutynin 5 mg PO TID for urge incontinence. She also takes hydrochlorothiazide for hypertension and has no known drug allergies. She reports dry mouth, new constipation and some mild concerns about her memory. On examination, visual acuity is normal in both eyes, there is mild bilateral conjunctival injection with a reduced tear film, no discharge and no photophobia, and the corneas are clear.
