A 41-year-old woman who works as an accountant comes to your clinic to establish care. Since adolescence she has had intense fear of being judged or humiliated in social situations. She avoids presenting at meetings, eating in front of colleagues and attending social events, and when she cannot avoid them she has blushing, sweating and palpitations. She has no panic attacks, depressed mood, substance use or history of seizures. For the past 3 years she has taken clonazepam 1.5 mg daily, prescribed by her previous physician, and she now wants to stop it because of daytime sedation and "mental fog". She takes an oral contraceptive and has no allergies. Physical examination and TSH are normal.
