An 8-month-old formula-fed girl is brought to a primary care clinic because of 5 days of hard, pellet-like stools passed every 3 to 4 days with straining and crying. Her mother noticed streaks of bright red blood on the surface of the stool and on the diaper. Rice cereal and vegetable and fruit purees were introduced 3 weeks ago. She passed meconium on the first day of life, is feeding well, has had no vomiting, and her weight is at the 50th percentile, in keeping with her previous visits. On examination she is afebrile and well appearing, her abdomen is soft and not distended, and there is a small posterior anal fissure with no other perianal abnormality.
