A 45-year-old man presents with a 1-week history of a sore mouth and an altered, unpleasant taste. He has had asthma for 10 years and uses a fluticasone propionate/salmeterol metered-dose inhaler (125/25 mcg), 2 puffs BID, without a spacer, plus salbutamol as needed. He has no other medical conditions, takes no other medications, and has no known drug allergies. He is eating normally and has no pain on swallowing. On examination, there are creamy white plaques on the buccal mucosa and tongue that can be scraped off, leaving erythematous mucosa; there is no lymphadenopathy.
