A 62-year-old man presents to the emergency department with a nosebleed from the right nostril that started 45 minutes ago. At home he leaned forward and pinched the soft part of his nose continuously for 15 minutes, but the bleeding continued. He has hypertension treated with ramipril, takes no antiplatelet or anticoagulant medication, and has no history of bleeding disorder or recent nasal trauma. He is alert, the airway is clear, and he is spitting small amounts of blood. On examination, the clots have been cleared, there is ongoing bleeding from the right anterior nasal cavity, and no discrete bleeding vessel can be seen. There is no blood draining down the posterior pharynx.
