An 82-year-old woman is on hospital day 2 for a urinary tract infection and dehydration, treated with ceftriaxone IV and IV fluids. An indwelling urinary catheter was inserted to monitor urine output, and oxybutynin was started yesterday for bladder spasms. Since last night she has been confused, drowsy by day, and agitated and sees children in her room at night. Her family reports mild memory problems at baseline, but she lives alone and is independent. On examination she is inattentive, cannot recite the months backward, has disorganized speech, and repeatedly pulls at her IV and catheter and tries to climb out of bed. Sodium is 138 mmol/L (135-145) and glucose is 6.2 mmol/L (4.0-7.8). Other medications are amlodipine and acetaminophen.
