A 30-year-old woman, 14 days postpartum after an uncomplicated term vaginal delivery, presents with 2 days of right breast pain, fever and chills. She exclusively breastfeeds her healthy 2-week-old term infant, who is feeding well and is not jaundiced. As a child she had urticaria and throat swelling after taking amoxicillin, and she takes no regular medications. She has had no hospitalization or antibiotics in the past year, no previous MRSA infection and no known HIV. On examination, there is a wedge-shaped, warm, tender, erythematous area in the upper outer quadrant of the right breast with a cracked nipple, and no fluctuance.
